I'm not sure of the answer to the first one. "Ad Asylum" took six weeks to write and two months to edit and rewrite, but it's based on 25 years of experience. So I'm not sure if it took 6 weeks or 25 years. How did I come up with the idea? When you're in the thick of writing something, you're not really thinking about how you got the idea. You're just doing it. But after you're done, and people keep asking you the same question, you can think back and try to figure out what you actually did.
